Referring to Health N Able

Health N Able welcomes referrals for adults who may benefit from Occupational Therapy services.

Whether you're a health professional, support provider, family member or referring yourself, we're happy to discuss how Occupational Therapy may assist.

Our aim is to make the referral process straightforward and provide timely communication throughout the assessment process.

Who We Support

Health N Able provides mobile Occupational Therapy services for:

Veterans (eligible Veteran Gold and White Card holders)

I

NDIS participants (self-managed and plan-managed adults aged 18 years and over)

I

Seniors and older adults living in the community

Private clients

Occupational Therapy Services

Health N Able provides assessment and recommendations in the following areas:

Home & Functional Assessments

Assessment of functional abilities, mobility, home safety and participation in everyday activities.

Assistive Technology

Assessment, prescription and education for assistive technology that supports independence, safety and participation.

Home Modifications

Assessment and recommendations for minor and major home modifications to improve accessibility, function and safety within the home environment.

Education & Training

Practical education for clients, families and carers to support the safe and effective use of equipment and everyday strategies.

What Happens After a Referral?

Once a referral is received:

We will review the referral information.

I

We'll contact the client (or their nominated representative) to discuss their needs and arrange an appointment.

I

An Occupational Therapy assessment will be completed in the client's home.

I

Recommendations will be discussed with the client and, where appropriate, a written report will be provided to the referrer.

Communication is maintained throughout the process to ensure recommendations are practical, appropriate and aligned with the client's goals.
